Update Contact Information

Washington Certification Services maintains each waterworks operator's contact information and notifies Department of Health of all changes.

It is your responsibility to notify Washington Certification Services of changes in home address, email address, employer or other contact updates. Most of the important waterworks certification program information is now distributed by email. Failure to provide a valid home and email address, or submit changes, does not constitute an excuse for waiver of the late fee assessment, failure to renew a certificate, or failure to demonstrate professional growth. Department of Health will not consider appeals from operators who did not provide and update home and email addresses.

If the information is not accurate, complete and submit the online Waterworks Operator Information Change Form.
